Our Referral Process — 5 Steps
- Complete our secure online referral form below, or download and email our PDF referral form to referrals@harbourofhope.com
- Our intake team will contact the referrer within 1 working day to acknowledge receipt and clarify any queries.
- An initial needs assessment is arranged within 48 hours of referral acceptance.
- A personalised support plan is developed in full collaboration with the individual and all relevant stakeholders.
- Placement is confirmed and a structured transition support plan is put in place.
